Referral Process

Anyone can make a referral for services. To be eligible for services an individual must have:

• a documented physical, mental or behavioral disability. Documentation must be a written diagnosis of a disability signed by a qualified professional, such as a medical doctor, psychologist, psychiatrist or licensed clinical social worker.
• funding for specific rehabilitation services, including facility-based services, community-based services and job placement services

All decisions regarding admission, retention, assignment and termination are based upon the needs of the individual and Linden’s abilities to provide needed services.
